Forms 101 and 108 use Adobe XFA technology for built-in calculations, validation, and dynamic form behavior.

These forms must be completed using Adobe Acrobat or Adobe Acrobat Reader. Web browsers and some PDF editors do not fully support XFA forms and may cause the forms to appear to work incorrectly.

• Form 108 may open in a browser and allow data entry, but calculations will not function.
• Form 101 will display a “Please wait…” message when opened in a browser. This message does not indicate a problem with the form.

To ensure proper functionality, download the form and open it directly in Adobe Acrobat or Adobe Acrobat Reader.
